Our Approach
We take a recovery-focused, trauma-informed approach. Every person we support has an individually tailored care and support plan developed collaboratively with them, their care team and relevant professionals.
Risk management is positive and enabling — we work to understand each person's strengths and goals, managing risk in a way that promotes progression rather than restriction.
Section 117 Aftercare
We have extensive experience working within Section 117 aftercare frameworks. We work closely with responsible commissioners, local authorities and NHS Trusts to ensure that all aftercare obligations are met and that the individual's entitlements are fully supported.
Referral Pathway
A clear, structured pathway from referral through to placement and ongoing support
Referral & Initial Screening
Referrals accepted from NHS teams, forensic services, Social Workers, Probation and Care Coordinators. We review the referral and confirm if we can meet the individual's needs.
Comprehensive Assessment
A thorough pre-placement assessment covering mental health, risk, independent living skills, housing and support needs — conducted in partnership with the referring team.
Transition Planning
A phased transition plan is developed with all relevant professionals, ensuring a safe, well-supported move into our accommodation.
Placement & Support
The individual moves into their new home with a bespoke support plan. Regular reviews ensure the placement continues to meet their evolving needs.
Ongoing Review & Progression
We work towards increasing independence over time, with clear milestones and pathways towards less restrictive, more autonomous living.
Who We Work With
We maintain strong, professional working relationships with the agencies responsible for the care, risk management and community reintegration of the people we support.
